后房型人工晶状体锯齿状“Z”形缝线固定术临床应用分析

目的探讨后房型人工晶状体锯齿状＂Z＂形缝线固定术的临床应用及效果。方法后囊破裂18例（18眼）进行锯齿状＂Z＂形缝线后房型人工晶状体固定术。随访3-6个月,平均4.5个月。结果 8例出现轻度角膜水肿及前房反应,3例出现瞳孔区渗出膜,1周后反应消失。1例出现轻度人工晶状体偏中心现象无须处理。1例玻璃体积血视力0.01,1例因青光眼视神经萎缩视力0.1外,0.2-0.4者10眼,0.5-0.6者5眼,0.8者1眼。术后无一例发生固定缝线松脱或囊泡,无结膜刺激症状或眼内炎发生。结论不使用线结而有效固定人工晶状体,可能减少巩膜坏死、缝线融解及感染,消除了手术线结导致摩擦症状等并发症。无须制作最后用于埋藏线结的巩膜瓣或巩膜槽,有利于患者术后早期视觉康复以及减少并发症的发生。

Objective Explore the jagged fonts ＋ z stitches after atrial artificial crystal fixed-Clinical application and effect. Methods In July 2007 to March 2009,We completed the 18 cases（18 eyes） of posterior capsule ruptured zigzag ＋ z-shaped suture of posterior chamber artificial lens fixation Surgery.Follow-up 3 to 6 months,an average of 4.5 months. Results 8 cases appeared mild corneal edema and anterior chamber reaction,and 3 cases of pupillary area appears exudative membrane,these phenomenon disappeared after a week.1 cases of mild artificial lens appears the phenomenon of off-Center,and not done deal,l cases of vitreous hemorrhage lead to vision to reached 0.01,one cases of glaucoma due to optic atrophy,and Visual acuity dropped to 0.l,ten case in the 0.2 to 0.4 between,five case in the 0.5 to 0.6 between,one case has reached 0.8.No case happened fix suture loose and Vesicles,post-operative patients with no symptoms of stimulate the conjunctiva and endophthalmitis occurred. Conclusion Don＇t use line knot fixed artificial lens effectively, Can reduce the sclera necrosis,suture lysis and infection,Can be elimination of patients with line knot leading to friction,and other symptoms of complications.At the same time,there is no need to buried knot line under scleral flap or scleral slot,that will help patients with early visual rehabilitation and to reduce complications.
