颈椎椎体转移瘤前路切除钛网植骨加钢板重建

Treatment of cervical metastatic tumor by anterior approach excision and repair with titanium mesh cage and bone graft combined with plate internal fixation

摘要 目的:探讨经颈前路切除颈椎椎体转移瘤,采用钛网植骨加钢板内固定重建椎体的临床应用价值。方法:行颈前路椎体切除,钛网自体髂骨植骨融合,钢板内固定治疗12例颈椎椎体转移瘤。结果:12例均获6~49个月,平均16.2个月的随访。全部病例植骨融合满意,局部肿瘤无复发,内固定物无断裂、松动、滑脱等并发症。伴有神经功能障碍者,其神经功能均有不同程度的恢复。结论:经颈前路切除转移瘤椎体,钛网植骨加钢板重建切除后椎体,可即刻保证固定节段的稳定性。植骨融合良好,明显改善神经功能, Objective:To investigate the clinical value of anterior approach excision decompression and repair with titanium mesh cage and bone graft combined with plate internal fixation in the treatment of cervical metastatic tumor.Methods:Twelve patients with cervical metastatic tumor were treated by anterior approach excision and repaired with titanium mesh cage,autologous iliac bone graft and plate internal fixation.Results:During the average follow-up period of 16.2 months(range,6-49 months),the solid fusion was satisfied in all patients,no local tumor recurrence was observed,and no complication occurred,including internal fixation breaking,loosing and olisthy.The nerve function in all patients with nerve dysfunction improved to some extent.Conclusion:The stability of fixed segment can be obtained immediately after anterior approach excision and repair with titanium mesh cage and bone graft combined with plate internal fixation,and the satisfactory bone graft fusion and improvement of nerve function can be obtained also.
