Accuracy of Blood Pressure Measurement Using Wrist Blood Pressure Monitor

摘要 目的应用腕式血压计测量间接血压与主动脉根部直接测量血压比较,探讨冠状动脉性心脏病(冠心病)患者采用腕式电子血压计测量的准确性。方法将冠脉造影患者(282例,男女比例为181∶101)分为冠心病高血压组86例、单纯冠心病组88例、单纯高血压组57例、正常对照组51例,将测得的腕式血压与中心动脉压通过组内相关系数行一致性评价;以腕式血压与中心动脉压的差值为指标,对高血压、冠心病两因素行析因设计资料的方差分析;同时用英国高血压学会(BHS)仪器设备评级标准对腕式血压计进行准确性评价。结果冠心病对腕式电子血压计收缩压的测量值与主动脉根部直接收缩压的测量值比较,差异有统计学意义(P<0.01);舒张压的测量值差异无统计学意义(P>0.05)。按BHS仪器设备评级标准,冠心病患者收缩压测量评级为D级,但舒张压测量评级为A级。无冠心病的单纯性高血压患者和血压正常患者,对腕式电子血压计收缩压和舒张压的测量值,与主动脉根部直接收缩压与舒张压的测量值的差异均无统计学意义。按BHS仪器设备评级均为A级。结论冠心病对腕式电子血压计收缩压的准确性有影响,而对舒张压无影响。 Objective To observe the accuracy of Wrist Blood Pressure Monitor in patients with Coronary heart disease(CHD).Methods Patients underwent Coronary angiography(282,men/women:181/101)were divided into CHD+hypertension(n=86),CHD(n=88),hypertension(n=57)and the control(n=51).The consistency of indirect wrist and invasive direct central aortic blood pressure were evaluated by the correlation coefficient;the value of difference between indirect wrist and direct aortic pressure was used as index.The accuracy of Wrist Blood Pressure Monitor was evaluated according to the British Hypertension Society(BHS)standard.Results There is significant difference between indirect wrist systolic pressure and direct aortic systolic pressure in patients with CHD(P0.01),however there was no difference of diastolic blood pressure between two methods(P0.05).According to BHS standard,the accuracy level of diastolic blood pressure monitoring was A,meanwhile the accuracy level of systolic blood pressure was only D in the CHD group.There is no significant difference of both systolic and diastolic blood pressure between the wrist and the aortic blood pressure measurement in both hypertension and the control group,accordingly,the accuracy based on BHS level was A.Conclusion Inaccuracy of systolic blood pressure measurement was found in CHD patients using electronic blood pressure monitor.
