超声下凝血酶注射治疗假性动脉瘤的疗效及护理

Treatment of Pseudoaneurysms by Ultrasound-guided Thrombin Injection and Nursing

目的探讨在超声下腔内注射凝血酶治疗假性动脉瘤的疗效及护理。方法对15例心脏冠状动脉介入术后并发假性动脉瘤患者采取超声引导下瘤腔内注射凝血酶栓塞治疗,并提供积极的护理措施。结果 12例患者一次性栓塞成功,有3例患者经2次栓塞成功,总成功率为100%。无动脉栓塞及静脉血栓形成、无凝血酶过敏表现、无感染、出血等并发症。结论超声引导下瘤腔内注射凝血酶的方法治疗假性动脉瘤简单、安全、成功率高。但在整个过程中充分的术前准备、娴熟的术中技术和细致的术后护理是手术成功的重要保障。

Objective To evaluate the therapeutic efficacy of ultrasound-guided thrombin injection in the treatment of pseudoaneurysms and its nursing.Methods Six male patients and 9 female patients with pseudoaneurysms after percutaneous coronary intervention（PCI） from January 2007 to December 2009 in our hospital were treated by ultrasound-guided thrombin injection,and positive nursing were provided fro them.Results Twelve patients had embolization at the first time,3 had embolization at the second time.The total successful rate was 100%.The doses of thrombin ranged from 200u to 800u.Three patients had a slight fever after thrombin injection,but they got resolved soon.There were no complications of arterial embolisms,venous thrombosis,thrombin allergy,infection or hemorrhage.Conclusion The application of ultrasound-guided thrombin injection to treat pseudoaneurysms is not only easy and safe but also highly successful,but the sufficient preoperative preparation,skillful performance and careful postoperative nursing are important to ensure the success of the operation.
