微螺钉或横腭杆支抗治疗双颌前突的临床疗效对比研究 被引量:12

Comparison of Treatment Outcomes between Miniscrew Implants and Transpalatal Arch Anchorage in Adults with Bimaxillary Dentoalveolar Protrusion

摘要 目的:研究以微螺钉种植体或横腭杆作支抗,治疗成人双颌前突患者,通过比较其颜面软硬组织变化,评价其临床疗效。方法:将26名平均年龄为20.96岁的双颌前突患者分为两组,分别以横腭杆(组1)、横腭杆结合种植钉(组2)为支抗,应用直丝弓技术行减数正畸治疗,对头颅侧位片行头影测量,运用统计学方法比较两组不同支抗术前术后软硬组织的变化差异。结果:组2的上唇美线距LS-EP、上颌磨牙水平位置U6-VP、上颌磨牙垂直位置U6-HP、上唇突距LS-VP、上切牙水平距U1-VP治疗前后的差值较组1显著减小;两组其他测量值差值无统计学意义。结论:横腭杆结合微螺钉作支抗较横腭杆能更多地内收上切牙并改善上唇突度,支抗作用更显著。 Objective:To evaluate the orthodontic effects of miniscrew implants and transpalatal arch(TPA)for anchorage in patients with bimaxillary protrusion malocclusion by comparing the changes of hard tissue and facial profile.Methods:Twenty-six patients aged 20.96 with bimaxillary dentoalveolar protrusion were devided into two groups.They were anchored by TPA(group1)and miniscrew implants and TPA(group2)respectively.Treated with the MBT system,four first premolars were extracted.The changes in skeletal,dental and soft tissues were analyzed in both groups on lateral cephalometric before and after active orthodontic treatment.Statistical methods were utilized to analyze the morphological changes of facial profile and hard tissues in both groups.Results:Compared with gorup1,significantly decreased differences of LS-EP,U6-VP,U6-HP,LS-VP,U1-VP betore and after treatment were observed in group2.The differences of other parameters changed nonstatistically.Conclusion:Using both mini-screw implants and transpalatal arch,anchorage were more effective and had greater retraction of the maxillary incisor and upper lip.
