降压药物对老年高血压患者动脉硬化的影响

The Influence of Antihypertensive Drugs on Arteriosclerosis in Elder Patients with Hypertension

目的:观察降压药物对老年高血压患者动脉硬化的影响。方法:选择80例原发性高血压患者随机分成两组,A组给予非洛地平缓释片5mg.天-1治疗,B组予厄贝沙坦150mg.天-1治疗,检测治疗前及3月后患者baPWV(肱踝脉搏波传导速度)指标。结果:1.两组患者治疗3月后血压均有下降,B组收缩压(126.13±7.99)mmHg较A组(131.38±5.23)mmHg低,差异有统计学意义(t=3.476,P<0.01);B组脉压(55.23±9.57)mmHg较A组(60.98±7.57)mmHg低,差异有统计学意义(t=2.98,P<0.01);2.高血压组baPWV(1734.71±471.14cm/s)明显较健康体检组(1435.01±211.69cm/s)高,差异有统计学意义(t=3.670,P<0.001)。两组患者治疗3月后baPWV均较治疗前下降,且两组治疗后比较,B组(1426.08±263.95cm/s)较A组(1586.60±396.45cm/s)降得更低,差异显著(t=2.487,P<0.05),B组△baPWV(306.85±259.26cm/s)较A组(132.45±138.42cm/s)大,差异显著(t=3.753,P<0.001)。结论:老年高血压患者动脉弹性减退,积极降压治疗3月后动脉弹性有一定程度恢复,且服用厄贝沙坦组患者动脉弹性较非洛地平缓释片组患者改善显著。

Objective To observe the alteration of arteriosclerosis in elder patients with hypertension who were treated by antihypertensive drugs.Methods Eighty elder patients with primary hyptertension were randomly divided into two groups.The patients in group A were given felodipine sustaind-release tablets five milligram per day,and the patients in group B was given irbesartan 150 milligram per day.Brachial-ankle Pulse Wave Velocity（baPWV）was detected in all patients before and after antihypertensive therapy three monthes.Eighty healthy volunteers who were older than sixty years were served as control group.Results 1.Blood pressure in both groups was lower after antihypertensive therapy.The systolic pressure and pulse pressure decreased more in group B than those in group A（126.13±7.99 mmHg vs 131.38±5.23mmHg,P0.01;and 55.23±9.57mmHg vs 60.98±7.57mmHg,P0.01）.And also for the decrease of pulse pressure,the patients in group B was more larger than that in group A（25.40±12.38mmHg vs 10.48±10.41mmHg,P0.01）.2.The Brachial-ankle Pulse Wave Velocity（baPWV）in elder patients with primary hypertension was more higher than that in healthy volunteers（60 years healthy people）（1734.71±471.14cm/s vs 1435.01±211.69cm/s,P0.001）.After antihypertensive therapy for three months,baPWV in both groups was decreased,but the patients in group B had got more lower level than in group A（1426.08±263.95cm/s vs 1586.60±396.45cm/s,P0.05）.The baPWV decreased more in group B than in group A（306.85±259.26cm/s vs 132.45±138.42cm/s,P0.001）.Conclusions There was diminished arterial elasticity in elder patients with primary hypertension,the arterial elastic got some recovered after 3 month＇s antihypertensive therapy.The arterial elasticity was improved more with irbesatan than with Felodipine sustaind-release tablet.
