电视纵隔镜下胸部疾病诊断63例护理配合

Nursing cooperation in the diagnosis of 63 cases of Chest disease with video-mediastinoscopy

目的:探讨电视纵隔镜下胸部疾病诊断手术护理配合的方法。方法:回顾性分析63例电视纵隔镜下胸部疾病诊断患者的护理资料。结果:本组15例怀疑或已经确诊为肺癌患者,淋巴结阳性率为60.0%(9/15),病理假阳性率16.7%(1/6)。48例纵隔疑难疾病患者中,47例获得病理诊断,1例临床证实,经验性治疗肿块明显缩小,疾病确诊率98%(47/48)。无手术死亡,无病理标本术中存放不当受损,1例大出血保守治疗成功。无其他相关术后并发症发生。结论:电视纵隔镜下手术对明确肺癌的分期和诊断纵隔疑难疾病检查准确率高,手术过程中要做好各期准备,积极预防术中并发症。

Objective： To evaluate the method of nursing cooperation in diagnostic surgery of chest disease with video-mediastinoscopy.Methods： The data of nursing care of 63 patients who received diagnostic surgery with video-mediastinoscopy were retrospectively analyzed.Results： The diagnosis showed that lymph node positive rate was 60.0%（9 / 15） and pathological false positive rate was 16.7%（1 / 6） in 15 patients who were suspected or had been diagnosed of lung cancer.47 of 48 patients with undifferentiated mediastinal disease were pathologically confirmed and the diagnosis rate was 98%（47/48）.No death case due to surgery and no intraoperative pathological specimens were damaged because of improper keeping.One patient with massive bleeding was given conservative treatment and successfully treated.No other complications occurred in the patients.Conclusion： The diagnostic surgery with video-mediastinoscopy has high accuracy in staging lung cancer and diagnosing undifferentiated mediastinal diseases.It is necessary to make full preparation for each period of the operation and actively prevent intraoperative complications.
