多原发性结直肠癌CT表现研究

CT manifestations of multiple primary colorectal carcinoma

目的探讨多原发性结直肠癌的CT表现特点。方法回顾性分本院2000-2009年收治61例多原发性结直肠癌患者的临床资料,影像资料及病理资料;结果本组61例共计131个癌灶,表现为肠管不规则增厚为18例,CT表现为管壁不规则增厚且强化较明显;43例表现为软组织肿块影,其中癌灶小于5cm者17例,表现为不均匀强化5例,表现为均匀强化2例;大于或等于5cm者26例,CT增强均表现为不均匀强化;19例近端癌灶出现肠梗阻,8例远端癌灶出现肠梗阻;所有61例病例均出现肠外浸润或转移,其中47例伴肠系膜淋巴结肿大;33例出现腹水;17例出现肝转移。结论 CT对多原发性结直肠癌的诊断及治疗有指导性意义。

Objective To study CT traits of the multiple primary colorectal carcinoma; Methods Retrospective 2000-2009 our hospital treated 61 cases of multi-year primary colorectal cancer patients with clinical data,imaging data and pathological data; Results There are a total of 61 foci in 27 cases in this group ,showing irregular thickening of the bowel in 8 cases,CT showed irregular wall thickening and strengthening the more obvious; 19 cases showed soft tissue mass,in which foci were less than 5cm 7 cases showed inhomogeneous enhancement in 5 cases,2 case showed homogeneous enhanced; more than 5cm in 12 cases,CT showed both enhanced non-uniform hardening; 10 cases of intestinal obstruction occurs proximal foci,and 3 distal foci Obstruction occurred; all 27 cases suffered from parenteral invasion or metastasis,of which 21 cases with mesenteric lymph nodes; 13 patients had ascites; 7 patients had liver metastases. Conclusion CT image has a guiding significance to multiple primary colorectal cancer diagnosis and treatment.
