带蒂颊脂垫移植在口腔黏膜缺损修复中的应用

The Clinical Application of Pedicled Buccal Fat Pad Reconstruction Oral Mucosa Defects

目的观察无覆盖带蒂颊脂垫组织瓣在口腔黏膜缺损修复中的临床应用效果。方法口腔组织病变切除或扩大切除术后,用无覆盖带蒂颊脂垫移植修复口腔黏膜缺损,对其愈后进行观察。结果本组13例,12例4～6周后颊脂垫完全上皮化,6个月后与正常黏膜类似,1例远端部分感染、坏死。其中1例轻度张口受限,1例中度张口受限,1例面颊凹陷。结论带蒂颊脂垫瓣血运丰富,抗感染能力较强,成活率较高,供区和受区切口隐蔽、美观。但带蒂颊脂垫瓣修复口腔组织有其局限性,对于较大的组织缺损不宜选择。

Objective The purpose of this study was to assess the clinical application of using of the buccal fat pad as a pedicled graft reparing oral mucosa defects.Methods We used the buccal fat pad as an uncovered pedicled graft to reconstruct the oral mucosa defects after resection of the oral mucosa lesions.Observe the clinical effects.Results 13 patients had performed the reconstruction of oral mucosa defects using the uncovered pedicled buccal fat pad graft.After the operation,most of the patients had good results.Only one patient had part necrosis of the buccal fat pad.one patients had slightly limited mouth open and one patient had moderately limited mouth open.one patients had slightly deformity of cheek depress.Conclusion we conclude that the use of the pedicled buccal fat pad graft for the reconstruction of oral mucosa defects in the mouth is worthy of consideration.The pedicled buccal fat pad graft was a safe,effective and cosmetically appealing approach for the moderately oral mucosa defects,but it can not apply in the huge oral mucosa defects.
