留置吸痰管并无创通气用于Ⅱ型呼吸衰竭探讨 被引量:3

Explore on the Usage of Non-Invasive Ventilation and Indwelling Endotracheal Suction Tube to the Therapeutic Effect of Ⅱ-Type Respiratory Failure Disease

摘要 目的探讨慢性阻塞性肺部疾病合并Ⅱ型呼吸衰竭的患者无创通气时,行气管内留置吸痰管的治疗效果。方法将符合入选条件的84例患者,随机分为对照组40例和观察组44例,两组均进行无创通气,对照组按常规方法进行经口或经鼻插入吸痰管吸痰,观察组的患者从鼻腔插入留置吸痰管固定在鼻翼上,吸痰管末端从面罩中穿出,按需吸痰,常规向吸痰管内滴入气道湿化液,比较两组患者的治疗效果。结果观察组患者治疗效果好、病死率低,使用有创通气的比例小,带呼吸机时间短,两组比较差异有统计学意义(P<0.05,P<0.01)。结论对慢性阻塞性肺部疾病合并Ⅱ型呼吸衰竭需要进行无创通气的患者,可以采用气管内留置吸痰管的方式按需吸痰,患者痛苦小、治疗效果好。 Objective To explore on the therapeutic effect of non-invasive ventilation and indwelling endotracheal suction tube to the patients with chronic obstructive pulmonary and Ⅱ respiratory failure disease.Methods 84 cases of patients were randomly divided the into control group(40 cases)and observe group(44 cases),non-invasive ventilation was performed in both of them,suctioning for the control group according to a conventional way-insert the suction tube through the mouth or nose;to the observe group,the suction tube was inserted from the nasal cavity and indwelling the suction tube and fixed on to the nose,the end of the suction tube piercing from the end of the mask,suctioning on demand,conventional instillation airway humidification solution into the suction piple,the different of therapeutic effect was compared between the two groups.Results The observe group had a better therapeutic effect and the case-fatality rate was low,also had the small proportion with the use of invasive ventilation and a shot time with ventilator and a short time with ventilator(P〈0.05,P〈0.01).Conclusion To the patients with chronic obstructive pulmonary patients and Ⅱ respiratory failure disease,can suctioning on demand with method of indwelling endotracheal suction piple,the patients will be less painful and the therapeutic effect is good demand with method of indwelling endotracheal suction piple,therapy will be less painful and the therapeutic effect is good.
