摘要
目的根据生物力学原理研制出一种治疗股骨颈骨折的新型组合固定器械(iliacfemurretractor、IFR).并对其进行生物力学测试和临床应用。方法取7具尸体髓关节标本,模拟股骨颈骨折.然后采用组台装置固定,对照组采用单纯加压螺纹钉固定,对比两组固定的生物力学强度和刚度。自1997年临床应用20例。结果实验组与对照组相比,应力一应变降低45%,强度裕度46%,位移相差16%。轴向刚度和侧向刚度分别提高32%和42%,抗扭强度相差34.7%,承载能力达原始骨的98%。20例中随访半年以上的16例,6例在3个月愈合,10例在半年内愈台。结论组合装置固定优于单纯螺纹钉内固定。
Objective To develop a new composite feature of extantal finator and threecompress screws for the patients with femoral nab fractllre and to make a biomechanital andclinic study on the feature. Methods Seven cadaveric hip joints were used in biomechanicalinvestigation. First, some data of normal hip joints were recorded, then, the speimens weredivided into two groupe and the control osttomy was performed to the model of femoral neckfracture. The test group was fixed by the comPOsite feature and the cofltrol group was fedby only three compress screws. Since 1997, 20 patted with femoral neck fracture have treated by the composite fixture. Results The distribution of stress, strain, displacement,rotation and ultimate strength were recorded, the results of the composite fixture in theexperimental group were stronger than that of the control group, and the clinic effect wasgood and reliable. Conclusion The stabilized effect of the composite feature is better than thatof screws. Therefore, it is woyth to popularize.
