ERCP致胆总管十二指肠损伤的诊治分析

Diagnosis and treatment of common bile duct and duodenal injury induced by ERCP

目的探讨经内镜逆行胰胆管造影(ERCP)致胆总管、十二指肠损伤的诊断和治疗措施。方法对2001年10月至2009年9月ERCP取石所致胆总管、十二指肠损伤5例的临床资料进行回顾性分析。结果本组胆总管损伤3例和十二指肠损伤2例均系行ERCP同时行十二指肠镜下乳头括约肌切开术(EST)所致。5例分别或同时经修补损伤、胆总管和十二指肠引流、胃肠吻合后治愈。结论早期诊断、合理治疗是ERCP致胆总管、十二指肠损伤能否治疗成功的关键。

Objective To explore the diagnosis and treatment of common bile duct and duodenal injury induced by endoscopic retrograde eholangiopancreatography（ERCP）. Methods The clinical data of 5 cases of common bile duct and duodenal injury induced by ERCP from Oct. 2001 to Sept. 2009 were retrospectively analyzed. Results All the 5 cases were cured by damage repair,common bile duct and duodenal drainage and gastrointestinal anastomosis separately or simultaneously. Five cases were injured by ERCP and EST simultaneous operation, of which,3 had common bile duct injury,2 had duodenal injury. Conclusion Early diagnosis and rational treatment are important to successfully cure common bile duct and duodenal injury induced by ERCP.
