乳腺MRI:真实还是谎言?

MRI of breast:real or fake?

在乳腺癌的检查中,MRI可以发现更多的潜在病灶,而这些病灶靠传统的影像检查可能会漏掉。但是,对于能否把MRI作为早期乳腺癌患者的一项常规检查尚未取得一致意见。近年来,一些回顾性研究分析表明,尽管MRI可以发现更多的病灶,但并没有提高患者的生存率。而且,由于多中心病灶的发现,导致更多的患者选择了改良根治术,而不是保乳手术。刊登在《临床医师肿瘤杂志》(CA Cancer J Clin)2009年第5期的一篇文章就针对早期乳腺癌MRI检查的临床意义做了综述分析。本文对其中的一些观点提出了自己的看法,认为在某些特定的乳腺癌患者中,MRI或许能使患者获益。

Magnetic resonance imaging （MRI） may detect additional foci of breast cancer that are occult on conventional imaging. However, there＇s no consensus on whether MRI should be introduced as a routine test in preoperative staging of the affected breast in women with early breast cancer. Some nonrandomized studies in the recent years have demonstrated that although additional loci of cancer can be detected by MRI, the clinical outcomes of the patients may not be improved. Furthermore, detection of additional cancer loci by MRI leads to more unnecessary, radical, breast surgery. This paper focuses on the clinical significance of MRI in breast cancer, which was discussed in a review published in CA： A Cancer Journal for Clinicians in 5th issue in 2009. The conclusion is that in certain group of breast cancer, MRI may be benefit.
