摘要
目的探讨鼻咽癌患者血红蛋白浓度与其放射治疗近期疗效及5年生存率的相关性。方法回顾性分析1991年12月至1992年12月经本院病理确诊并进行系统治疗的鼻咽癌患者314例。全部病人予Co60常规分割根治性放疗,鼻咽病灶放疗剂量65~80/Gy/6~8W,颈部转移淋巴结放疗剂量60~70Gy/6~7W。放疗前及放疗过程中定期检测血红蛋白浓度。结果鼻咽癌患者放疗后的近期效果与其血红蛋白浓度无明显相关性。当血红蛋白浓度在80g~、90g~、100g~、110g~、120g~/L时,患者5年生存率分别为11.1%、36.4%、37.4%、37.9%、60.0%,组间差异有高度统计学意义(P<0.001)。结论血红蛋白浓度水平与鼻咽癌患者放疗后的远期疗效明显相关,5年生存率随着血红蛋白浓度的增高而升高。
Objective To investigate the correlation of peripheral blood hemoglubin level with the short-term therapeutic effect and 5-year survival rate of patients with nasopharyngeal carcinoma (NPC) treated by radiotherapy. Methods A retrospective study was carried out among 314 NPC patients treated in our Hospital from Dec, 1991 to Dec, 1992, with their clinical data carefully analyzed for the study. All these patients were given radical radiation therapy with the procedures of routine fractionation by Co^60 external beam. The dose was 65-80Gy/7-8w for nasopharyngeal focus and 60-70Gy/6-7w for metastatic cervical lymph nodes. Meanwhile, the levels of peripheral blood hemoglubin were measured among all these patients before and during the progress of radiotherapy periodically to observe its impact in the therapeutic effect for the cases. Result It was shown that there was no significant association of the hemoglobin level with the short-term therapeutic effect, while the 5-year survival rate of this group of NPC patients was obviously correlated with their hcmoglubin concentrations (P〈0.01), with the 5-year survival rates beingll.l%, 36.4%, 37.9% and 60.0% for those with the hemoglubin levels being 80-, 90-, 100-, 110- and 120-/L respectively when analyzed in a sub-grouping way. Conclusion The level of peripheral blood hemoglobin is significantly correlated with the long-term therapeutic effect of NPC patients treated by radiotherapy as indicated by 5-year survival rate, with a linear association of hemoglubine level with the 5-year survival rate.
