早期康复训练对脑出血术后患者肢体功能的影响

Effect of Early Rehabilitative Training on the Function Recovery of the Postoperative Patients with Hypertensive Intracerebral Hemorrhage

目的:探讨早期康复训练对脑出血术后偏瘫患者肢体功能恢复的影响。方法:高血压脑出血术后偏瘫患者66例,随机分为训练组和对照组各33例,均按脑出血术后常规治疗及护理。训练组配合系统的康复训练,包括早期良肢位保持及床上训练、痉挛期体位转换及恢复期步态及行走平衡训练,训练中针对患者心理状态,给予必要的心理支持。治疗前后2组均采用Fugl-meyer(FMA)评定运动功能;改良Barthel指数评定ADL能力。结果:治疗4周时,2组Barthel指数和FMA评分均较治疗前明显提高(P<0.05,0.01),训练组高于对照组(P<0.05)。治疗8周时,训练组各项指标评分与4周时比较继续上升(P<0.01),对照组则上升不明显。结论:早期系统的康复训练对高血压脑出血术后偏瘫患者肢体运动功能的恢复有显著促进作用。

Objective：To explore the effect of early rehabilitative training on the function recovery of the paralytic limb in postoperative patients with hypertensive intracerebral hemorrhage.Methods：Sixty cases of the paralytic limb in postoperative patients with hypertensive intracerebral hemorrhage were selected and randomly divided into rehabilitation group（n=33 cases）and the control group（n=33 cases）.Fugl-meyer assessment was used to assess the motor function in patients,and the modified Barthel index was used to assess the daily self-care ability in patients.The control group received the routine postoperative treatment and care.The patients in rehabilitation group were treated with early rehabilitation exercises besides the routine postoperative treatment and care.Barthel index and Fugl-meyer score were compared at the 4th and 8th week after treatment between two groups.Results：At the 4th week after treatment,Barthel index and FMA score in both two groups were significantly increased as compared with those before treatment,and those in rehabilitation group were higher than in control group（P0.05,0.01）.At the 8th week after treatment,the Barthel index and FMA score in rehabilitation group were continuously increased as compared with those at the 4th week（P0.001）.Conclusion：Early rehabilitation can significantly improve motor function of paralytic limb and activities of daily living capacity in postoperative patients with hypertensive intracerebral hemorrhage,and can improve their quality of life.
