冠心病不同中医证型与血浆纤维蛋白原的相关性研究 被引量:1

Relationships between Plasma Fibrinogen in Coronary Heart Disease with Various TCM Syndromes

摘要 目的:检测冠心病中医各证型患者血浆纤维蛋白原(FIB)浓度,为冠心病的中医辨证分型及辨证论治提供参考。方法:179例冠心病患者(冠心病组)按中医辨证分为心血瘀阻29例、痰浊内阻27例、阴寒凝滞33例、心肾阴虚30例、气阴两虚33例及阳气虚衰27例共6型,与排除冠心病的体检人员(对照组)30例均进行血浆FIB浓度测定。结果:与对照组比较,冠心病组患者FIB浓度均值明显升高(P<0.05);冠心病组中各证型间比较,阳气虚衰型FIB浓度>阴寒凝滞型>痰浊内阻型>气阴两虚型>心血瘀阻型>心肾阴虚型。阳气虚衰型急性冠脉综合征,其FIB阳性率明显高于其他各证型(P<0.05)。结论:FIB浓度的变化可能与冠心病严重程度有一定关系,在不同证型中阳气虚衰型FIB浓度增高其临床指导意义更大。 Objective:To provide an objective proof for syndrome differentiation of traditional Chinese medicine(TCM)and guide clinical treatment and syndromes in coronary heart disease(CHD).Methods:The patients with CHD were divided into 6 groups:heart blood stasis,retention of phlegm in heart vessel,stagnation of qi due to cold,heart-kidney yin deficiency,dual deficiency of qi and yin,dual deficiency yang and qi.Thirty healthy volunteers randomly selected from the medical center served as control group.The level of serum fibrinogen was determined.Results:The serum fibrinogen level in dual deficiency yang and qi group was statistically higher than in control group,and the groups of heart-kidney yin deficiency,heart blood stasis,dual deficiency of qi and yin(P0.05).The serum fibrinogen level in dual deficiency yang and qi was the highest,followed by stagnation of qi due to cold,retention of phlegm in heart vessel,dual deficiency of qi and yin,heart blood stasis,heart-kidney yin deficiency.The serum fibrinogen level in dual deficiency yang and qi was significantly higher than in control group,heart-kidney yin deficiency group,heart blood stasis group and dual deficiency of qi and ying group(P0.05).Conclusion:The changes of serum fibrinogen level may be related to the severity of CHD.
