经直肠超声造影对前列腺结节的诊断价值 被引量:2

The value of contrast-enhanced transrectal ultrasonography in diagnosis of prostate nodules

摘要 目的评价经直肠超声造影(CETRUS)对前列腺结节良恶性的诊断价值。方法应用CETRUS匹配成像技术(CnTI)及SW造影分析软件,记录40例患者共47个前列腺结节的强度增量(a1)、峰值减半斜率(a2)、上升斜率(a3)、灌注时间(t0)、达峰时间(TTP)、峰值强度(PI)、显影时间(AT)、曲线下面积(AUC)、强度增幅(AMP)、平均通过时间(MTT)等参数,比较良、恶性结节组间相应参数的差异。并对每个结节作受试者工作特征(ROC)分析。结果 25个前列腺恶性结节和22个前列腺良性结节a1分别为(28.49±6.06)和(18.26±9.19)(P<0.05),PI分别为(26.31±7.53)和(19.24±8.35)(P<0.05),AMP分别为(20.47±5.55)和(9.70±6.02)(P<0.05),MTT分别为(48.90±12.26)和(58.79±9.48)(P<0.05)。结论 CETRUS对前列腺结节良恶性的鉴别提供可靠信息。 Objective To evaluate the value of contrast-enhanced transrectal ultrasonography(CETRUS)in diagnosis of prostate nodules.Methods Contrast tuned imaging(CnTI)technique and SW quantitative software were used for analyzing 25 maligment prostate nodules(group M)and 22 benign nodules(group B).The parameters of the increase in the intensity(a1),decent curvature(a2),ascendant curvature(a3),time of perfusion(t0),time to peak intensity(TTP),peak intensity(PI),arriving time(AT),area under the curve(AUC),amplitude(AMP),mean trasit time(MTT)in each prostate nodule were recorded.Then the differences of these parameters in the benign and cancerous nodules were analyzed.Receiver operating characteristic(ROC)analysis was performed.Results The values of a1,PI,AMP and MTT were all higher in group M than those in group B[(28.49±6.06)vs.(18.26±9.19),(26.31±7.53)vs.(19.24±8.35),(20.47±5.55)vs.(9.70±6.02)and(48.90±12.26)vs.(58.79±9.48)](P0.05).Conclusion CETRUS can provide reliable information for differentiating the malignant nodule from the benign one of the prostate.
