养阴生津、凉血活血法早期介入治疗乙型肝炎肝硬化凝血功能紊乱的临床观察

Clinical observation on early traditional chinese medicine therapy on Coagulation disorders in liver cirrhosis caused by hepatitis B

目的探讨乙型肝炎肝硬化凝血功能紊乱的中西医结合治疗方案。方法随机抽取乙型肝炎肝硬化凝血功能紊乱的患者145例。研究组(70例)采用养阴生津、凉血活血法中医药辨证论治联合西医综合治疗,对照组(75例)采用传统西医综合治疗,治疗2～3个疗程,观察两组患者凝血酶原时间(PT)、纤维蛋白原(Fib)、血小板(PLT)等指标的变化情况以及出血事件的发生情况。结果研究组Fib、PT等指标的改善、出血事件的控制均优于对照组;研究组凝血功能的改善总有效率为67.1%,对照组为52%,两组比较均有统计学意义(P<0.05或P<0.01)。结论养阴生津、凉血活血法早期介入治疗乙型肝炎肝硬化凝血功能紊乱,能较好的改善凝血功能,减少出血事件的发生,具有较好的安全性。

Objective To discuss new therapy on coagulation disorders in liver cirrhosis caused by hepatitis B through the combination of traditional Chinese medicine（TCM）and Western medicine.Methods Total of 145 patients with liver cirrhosis caused by hepatitis B who complicated coagulation disorders were selected randomly.Seventy patients in research group were treated with TCM by ＂nourishing yin,cooling blood and invigorating blood circulation＂ and Western medicine,75 patients in control group were treated with conventional Western medicine.Both groups were treated for two to three courses.The indexes of prothrombin time（PT）,fibrinogen（Fib）,platelet（PLT）and bleeding events were observed and compared.Results Indexes of PT,Fib and bleeding events in the research group were all better than that in the control group.The overall effective rate of coagulation function in the research group and control group were 67.1% and 52%,respectively.All the differences were statistically significant（P0.05 or P0.01）.Conclusions According to the academic TCM theory of ＂preventive treatment of disease＂,early treatment by ＂nourishing yin,cooling the blood and invigorating blood circulation＂ can get greater therapeutic effect on improving the coagulation function and reduce bleeding events with high safety.
