阿托伐他汀对冠心病患者血脂的影响及各因素对LDL-C达标率影响的分析

Effect of atorvastatin on serum lipids in patients with coronary artery disease and analyze the factors on lipoprotein-cholesterol goal achievement rate

目的观察阿托伐他汀(降血脂药)对冠心病(CHD)患者血脂的影响并分析对低密度脂蛋白胆固醇(LDL-C)达标率的影响因素。方法 146例LDL-C未达标的CHD患者,口服阿托伐他汀20mg,晚1次,疗程8周,判断其疗效,并观察年龄、性别、体质量指数、膳食控制、糖化血红蛋白(HbA1c)等对LDL-C达标率的影响。结果治疗8周后,总胆固醇(TC)、LDL-C水平较服药前明显下降,差异有统计学意义(P<0.05)。年龄小于50岁、合理控制膳食、血糖控制良好的CHD患者,LDL-C达标率较高。多元Logistic逐步回归分析显示,膳食控制、HbA1c水平是CHD患者LDL-C达标率的独立影响因素。结论阿托伐他汀有效减低CHD患者LDL-C水平,饮食控制及HbA1c水平影响LDL-C达标率。

Objective To observe the effect of atorvastatin on serum lipids in patients with coronary artery disease and analyze the factors on lipoprotein - cholesterol （ LDL - C ） goal achievement rate . Methods One hundred and forty - six patients with coronary artery diseases whose LDL - C didn＇t reach the taget level were administrated with atorvastatin 20 mg daily for 8 weeks. Therapeutic effects were observed and the effects of age,gender,body ross index （BMI）,alimentary control,glycated hemoglobin HbA1c on LDL - C goal achievement rate of atorvastatin were ana- lyzed. Results After 8 weeks,total cholesteol （TC）, LDL - C levels significantly decreased compared with before treatment, the difference was statistically significant （P 〈 0. 05）. The achievement rate of LDL - C in patients （ 〈50 age） was higher than patients （ 〉70 age）. There was significant difference between the two groups （P 〈 0. 05）. The diet controlled group showed significantly higher rate in control of LDL - C than the diet uncontrolled group. The patients with well blood glucose controlled were better than the patients with bad blood glucose controlled. Logistic stepwise regression analysis showed that variables entered model were diet control and value of HbA1c. Conclusion Diet control and value of HbA1c were the independent risk factors of atorvastatin in patients with coronary artery disease.
