摘要 目的:探讨与冠状动脉慢血流(SCF)现象相关的危险因素。方法:回顾性分析2005年8月至2008年12月我院行冠状动脉造影显示心外膜主要冠状动脉无明显病变的病例,共186例的资料。以冠脉血流≤TIMI2级的为SCF患者,被分为SCF组(96例),TIMI3级者为对照组(90例)。结果:SCF组的人体质量指数[(27.06±3.40)kg/m2:(25.53±3.57)kg/m2];红细胞压积[(0.43±0.07):(0.41±0.04)],凝血酶时间[(18.23±2.80)s:(17.33±1.73)s],白细胞计数[(6.65±1.73)×109/L:(6.05±1.56)×109/L]均明显高于对照组(P均<0.05)。高密度脂蛋白-胆固醇明显低于对照组[(1.12±0.28)mmol/L:(1.21±0.29)mmol/L,P<0.05]。Logistic分析表明,BMI(OR1.373,95%CI1.146~1.645,P=0.001)、凝血酶时间(OR1.348,95%CI1.056~1.722,P=0.017)、白细胞计数(OR1.920,95%CI1.234~2.987,P=0.004)是发生SCF的危险因素。结论:体质指数、凝血酶时间、白细胞计数增高可能是促进冠状动脉慢血流发生的危险因素。 Objective: To analyze the correlation factors of slow coronary (low (SCF) retrospectively. Methods: The study population included 186 patients with angiography proven, their coronary arteries were normal. Coronary flow was determined by thrombolysis in myocardial infarction (TIMI) flow grade. Patients who belonged to ≤TIMI 2 were included in SCF group (n=96) and belonged to TIMI 3 were included in control group (n=90). Results: Body mass index [ (27.06±3.40) kg/m^2 vs. (25.53±3.57) kg/m^2], hematocrit [ (0.43±0.07) vs. (0.41±0.04)], thrombin time [ (18.23±2.80) s vs. (17.33±1.73) s], number of white blood cell [ (6.65±1.73)×10^9/L vs. (6.05×1.56) ×10^9/L] were significantly higher in SCF group compared with the control group respectively, P〈 0.05 all. But HDL-C level was significantly lower in SCF group than that of control group [ (1. 1220.28) mmol/L vs. (1 . 2120. 29) mmo l /L, P〈0. 05]. Logistic regression test suggest that, body mass index (OR 1. 373, 95% C1 1.146-1. 645, P=0.001), thrombin time (OR 1.348, 95% CI 1. 056-1. 722, P=0.017), white blood cell (OR 1. 920, 95% CI 1. 234-2. 987, P= 0. 004) were the risk factors of slow coronary flow. Conclusions: Body mass index, thrombin time, and number of white blood cell increase may be the risk factors of slow coronary flow.
