盆底肌电刺激治疗儿童神经性尿频临床评价

摘要 目的评价盆底肌电刺激治疗儿童神经性尿频的临床效果。方法临床表现为白天尿频的患儿17例,男12例,女5例,年龄3~5岁。尿常规检查排除泌尿系感染,排泄性膀胱尿道造影及B超检查排除泌尿系畸形,尿流动力学检查所有患儿存在逼尿肌无抑制收缩。应用盆底肌电刺激治疗进行治疗。结果治疗3~5天后,15例排尿间隔在2 h以上,随访3个月无复发,2例无效。尿流动力学检查表明15例逼尿肌无抑制收缩消失,1例逼尿肌无抑制收缩减轻,1例无变化。与治疗前比较,治疗后神经性尿频患儿膀胱容量、膀胱顺应性明显增加(P〈0.05),但功能性尿道长度无明显变化(P〉0.05)。结论盆底肌电刺激可抑制逼尿肌的收缩,增加膀胱容量,从而达到治疗效果。 Objective To investigate clinical effects of electrical stimulating pelvic floor musles to treat psychogenic urinary frequence in children.Methods There were 17 children with day time urinary frequence(male:12 cases,females:5 cases,age:3~5 years old).Infection and malformation were excluded by urinary retine detection,voiding cystoureterography and ultrasonography.Urodynamic found detrusor unstability in all patients.All patients were treated by electrical stimulating pelvic floor musles pulse rate:10Hz,pulse width:200μs,pulse intensity:6~8mA.They were trained for 15 minutes every day.Results 15 patients had good results after 3~5days treatment.Micturition intervals were above 2 hours.They were followed up for 3 months and had normal micturition without recurrence.2 cases had no change.The bladder volume and bladder compliance after treatment was better than before treatment(P0.05),but length of functional urethra wasn't remarkable change(P0.05).Conclusion Electrical stimulating pelvic floor musles has good effects to psychogenic urinary frequence in children.
