中药内服外洗治疗HAART所致外周神经损害的临床研究

Observation of the clinical efficacy of oral and washing herbs for peripheral nerve damage caused by HAART

目的观察中医药内服外洗对艾滋病(AIDS)抗病毒药所致外周神经损害的临床疗效。方法对经高效抗反转录病毒治疗(HAART)所致外周神经损害的AIDS病人35例,给予独活寄生汤加减内服,配合中药自拟艾麻退痹汤煎汤足浴外洗,治疗两个疗程。治疗前后主要观察病人症状体征积分、生活质量、四肢肌力、反射感觉缺失、感觉异常等外周神经损害变化。结果治疗后病人症状体征积分下降,有效18例,稳定7例,无效8例,总有效率为77.14%。卡氏积分疗前为(76±6.28)分,疗后为(83±6.53)分,较疗前有提高。外周神经损害症状变化,显效22例,有效9例,无效4例,总有效率为88.57%。表明治疗能够明显减轻病人症状体征,提高病人的生存质量。结论中医药内服外洗,对AIDS抗病毒药所致外周神经损害有较好的临床疗效,值得进一步观察应用。

Objective To observe the clinical efficacy of oral and washing herbs for peripheral nerve damage caused by the anti-retroviral medicine. Methods Thirty five AIDS patients with peripheral nerve damage caused by HAART were given orally Duhuojisheng decoction in combination with Aimatuibitang,a foot washing prescription for two courses,Patient,symptoms and signs,including muscle strength,sensory reflex and overall quality of life were observed before and after the treatment. Results After the treatment ,the scores of patients＇ symptoms and signs decreased,and the scores of their quality of life were raised from 76±6.28 to 83±63.35.The treatment was effective in 18 cases,7 cases with stable outcomes,and was uneffective in 8 cases ;the overall rate of efficacy was 77.14%.So far as the peripheral nerve damage was concerned evident improvement was seen in 22 cases,improvement in 9 cases,and no improvement in 4 cases. All this indicated that patients＇ symptoms,signs and quality of life were significantly improved after the treatment. Conclusions Oral and washing prescripitions of Chinses traditional herbs show relatively good efficacy for the improvement of peripheral nerve damage caused by HAART and should be further observed and applied.
