组织蛋白酶D表达与乳腺癌的关系

CATHEPSIN D AND PROGNOSIS IN BREAST CANCER

目的研究组织蛋白酶Ｄ（Ｃａｔｈ┐Ｄ）对乳腺癌侵袭、转移和预后的作用。方法采用免疫组织化学亲和素┐生物素┐过氧化物酶复合物（ＳＡＢＣ）法，检测１０５例乳腺癌石蜡切片中肿瘤细胞（实质）和间质细胞组织蛋白酶Ｄ的表达，结合临床和病理资料分析其作用。结果组织蛋白酶Ｄ主要定位于细胞浆，部分增生的细胞可见定位于细胞核，间质细胞与实质细胞的Ｃａｔｈ┐Ｄ的定位基本相似。间质细胞以肿瘤周围的间质细胞组织蛋白酶Ｄ的表达量为高。Ｃａｔｈ┐Ｄ在乳腺癌实质和间质中总的表达率分别为４９．５％（５２／１０５），５６．２％（５９／１０５）。实质Ｃａｔｈ┐Ｄ的表达与肿瘤细胞组织浸润、肿瘤大小、腋淋巴结转移、随访结果有关（Ｐ＜０．０５）；与乳腺癌组织学分类、组织学分型、分级、年龄、ＥＲ、ＰＲ状态、局部复发、远处转移无关。间质Ｃａｔｈ┐Ｄ的表达与乳腺癌肿瘤大小、腋淋巴结转移、局部复发、远处转移、年龄、ＥＲ、ＰＲ状态、局部复发无关。实质与间质Ｃａｔｈ┐Ｄ的表达和乳腺癌患者的无瘤生存率有关（Ｐ＜０．０５）；实质Ｃａｔｈ┐Ｄ表达与总生存率无关（Ｐ＞０．０５），而间质的Ｃａｔｈ┐Ｄ与总生存率有关（Ｐ＜０．０５）。结论实质和间质乳腺癌组织蛋白酶Ｄ的表达都和?

Objective:To study the effect of Cathepsin D(Cath D) on breast cancer invasion, metastasis and prognosis.Methods:By means of ABC test, 105 breast cancer specimens in paraffin sections were examined for the expression of Cath D.Results:Cath D protein was detected in cancer cell cytoplasm;the expression rates in cancer cells and in stromal cells were 49.5% (52/105) and 56.2%(59/105) respectively.Expression of Cath D in cancer cells was correlated to its invasiveness,bigger tumor size,higher axillary lymph node involvement and poorer prognosis( P <0 05),but not with those pathologic typing,grading,age,ER and PR status,local recurrence and distant metastasis.Expression of Cath D in stromal cells was associated with bigger tumor size,axillary lymph node metastasis,regional recurrence,distant metastasis and poorer prognosis( P <0 05) Conclusion:The neoplastic and stromal cathepsin D are neglected immunohistochemical prognostic parameters.They play a pivotal role in the invasion and breast cancer metastasis.It shows that cathepsin D,specially stromal catepsin D,induces breast cancer metastasis and influences the prognosis of patients.
