糖尿病视网膜病变患者生存质量及其医学影响因素调查

THE QUALITY OF PATIENTS'LIFE WITH DIABETIC RETINOPATHY AND ITS MEDICAL INFLUENCING FACTORS

目的评价糖尿病视网膜病变(diabetic retinopathy,DR)患者的生存质量及其医学影响因素,为DR筛查、诊断和治疗方案的选择提供流行病学依据。方法应用视功能损害患者生存质量量表对不同病变程度的160例DR患者进行测量分析。结果 DR对患者精神心理的影响最大,而对身体机能的影响最小。生存质量的各纬度均与患者视力状况呈正相关,与患者年龄、增殖性病变程度、黄斑水肿、DM病程及中心视野呈负相关。视力、黄斑病变是影响症状与视功能、身体机能、社会与精神心理活动以及生活质量总评分的主要因素。结论 DR黄斑病变是影响患者生存质量的主要医学因素,早期发现、诊断和治疗DR黄斑病变可改善DR患者的生存质量。

Objective To evaluate the quality of patients＇life with diabetic retinopathy （DR） and its medical influencing factors, and try to provide epidemiology evidence for DR screening, diagnosis and treatment. Methods The quality of 160 patients＇ life with DR was measured by the form of quality of patients＇ life with vision functional lesion. Results DR has the most influence on spiritual and psychological status of diabetic patients, while it has the less influence on physical function. The questionnaires of the quality of patients＇ life with DR has a positive correlation with the visual acuity. And it has a negative correlation with the patient＇ s age, the degree of the proliferative retinopathy, the macular edema, the course of the disease, and the central visual field. Visual acuity and macular disorder are the main influence factors on sympotom and visual function, physical function, social life and spiritual and psychological status and the score of the puality of life. Conclusion Maculopathy is the major medical factors for quality of life of patients with DR, early detection, early diagnosis and early treatment can improve the quality of patients＇ life with DR.
