断指再植术后血管危象的相关分析及对策

Study on postoperative vascular crisis of phalangeal fractures with mini-plate fixation.

目的分析断指再植术后造成血管危象发生的多种因素。方法回顾性分析2005年1月至2009年6月断指再植病例65例共81指病例资料,探讨断指再植发生血管危象的相关因素。结果本组65例81指中,发生血管危象12指(发生率为14.8%),其中动脉危象8指(发生率为66.7%),静脉危象4指(发生率为33.3%)。结论血管危象的发生多与疼痛、吸烟、寒冷及神经生理因素等有关,正确预防这些因素是降低血管危象的发生率,提高断指再植成功率的重要方法。

Objective To analyze various factors influencing the occurrence of vascular crisis after digital replantation. Methods The clinical data of 65 cases of finger replantation （ altogether 81 fingers） during January 2007 to June 2009 were retrospectively analyzed in order to explore the relative factors for occurrence of vascular crisis after finger replantation. Results Among 65 patients with 81 fingers in this series, vascular crisis occttrred in 12 fingers with the incidence of 14.8%, including arterial crisis in 8 of 12 fingers and venous crisis in 4 of 12 fingers, and the total incidence rate of crisis was 14.8%. Conclusion The occurrence of vascular crisis is closely related to pain, smoking, cold and neurophysiological factors. The correct measures for prevention of these factors may reduce the incidence of vascular crisis, and it is very important in raising successftil rate of replantation of fractured fingers.
