摘要
目的探讨实时超声造影对胰腺肿瘤的诊断价值。方法比较41例胰腺占位的超声造影表现,包括导管腺癌26例,淋巴瘤1例,侵袭性纤维瘤病1例,实性假乳头状瘤3例,神经内分泌肿瘤8例,自身免疫性胰腺炎2例。比较不同病理类型胰腺肿瘤与正常胰腺实质相比的增强强度变化。结果导管腺癌与交界性肿瘤及自身免疫性胰腺炎相比,增强方式的差异有统计学意义,若以早期与胰腺实质同步增强随后减退为低回声或者增强始终低于胰腺实质这两种增强模式作为诊断导管腺癌的依据,则其诊断的敏感性,特异性,准确性,阳性预测值,阴性预测值分别为92.3%,80.0%,87.8%,88.9%,85.7%。结论实时超声造影对胰腺导管腺癌的鉴别诊断有重要意义。
Objective To investigate the value of contrast-enhanced ultrasound in solid neoplasmas of pancreas.Methods Performance of 41 pancreatic neoplasms were compared on contrast-enhanced ultrasound（CEUS）,including 26 ductal adenocarcinomas,1 lymphoma,1 aggressive fibromatosis,3 solid-pseudopapillary tumor（SPT）,8 neuroendocrine tumors and 2 autoimmune pancreatitis（AIP）.Changes of contrast-enhanced ultrasoud intensity were compared with pancreas parenchyma between different pathological types of pancreas.Results Enhanced pattern had statistical significant difference in pancreatic ductal adenocarcinomas comparing with borderline tumor and AIP of pancreas.The sensitivity,specificity,accuracy,positive predictive value and negative predictive value were 92.3%,80.0%,87.8%,88.9%,85.7%,if the enhanced patterns of iso-enhanced to hypo-enhanced or persistent hypo-enhanced compared with pancreas were regarded as evidence for pancreatic ductal adenocarcinoma.Conclusion It has important value for difference dignosis of pancreatic ductal adenocarcinoma with contrastenhanced ultrasound.
