摘要
目的:探讨慢性心力衰竭(CHF)患者的管理模式对疾病预后的影响。方法:采集既往曾有反复住院史的慢性心衰患者36例,其中男性24例,女性12例,平均年龄61±12岁。心功能III级21例,IV级15例。在规范心衰一般治疗和药物治疗的同时,对患者进行有效的教育和沟通指导,让病人学会自我管理。结果:观察1年,患者的总住院次数、总住院日、平均每次住院日、急诊入院次数均下降,生活质量明显提高。结论:建立有效的康复管理模式对慢性心力衰竭患者的预后有着很重要的意义,值得进一步探索和推广。
Objective：Explore the influence of the management of patients with chronic heart failure（CHF） on prognosis of the disease.Method：Collect36 cases of patients with chronic heart failure that have a repeated hospitalization history.24 cases were males and 12 females with a mean age of 61 ± 12.21 cases of cardiac function grade Ⅲ and 15 cases of cardiac function grade Ⅳ.During the standard heart failure therapy and drug therapy,give patients effective education and communication guidance.So that patients learn to selfmanagement.Result：The patients＇ total hospitalization、total length of stay、average length of stay、emergency incident rate are decreased during the one year.and the quality of life are markedly improved.Conclusion：To establish an effective management model for the chronic heart failure patients have a very important significance,it is worthy of further exploration and promotion.
