经阴道彩超引导介入治疗早期输卵管妊娠的体会

Experience of Interventional Therapy Guided by Transvaginal Color Doppler Ultrasound for Early Tubal Pregnancy

目的探讨阴道彩色多普勒介入治疗早期输卵管妊娠的应用价值。方法选择经阴道彩超诊断早期未破型输卵管妊娠50例,经阴道对孕囊进行穿刺,并注入氨甲蝶呤。结果 50例早期未破型输卵管妊娠病例中,一次性介入治疗的治愈率达84%,有效率达10%,无效占6%,对5例有效患者均作了第2次或第3次介入治疗后总治愈率达94%,3例无效患者改为手术治疗。结论介入治疗成功率高,并发症极少,创伤小,可重复,费用低廉,是一种相对安全、简便、可靠的妇科治疗手段,对曾患输卵管妊娠而手术或未生育渴望保留生育能力的患者,开辟了一条新的治疗途径。

[Objective]To study the application value of interventional therapy guided by transvaginal color doppler ultrasound in the treatment of early tubal pregnancy.[Methods]50 cases of early unruptured tubal pregnancy diagnosed by transvaginal color doppler ultrasonography were selected,and the transvaginal puncture of gestational sac was performed to inject methotrexate.[Results]After first interventional therapy,the cure rate of 50 cases was 84%,the effective rate was 10%,and 6% of cases were ineffective. After 5 effective cases were given the second or third interventional therapy,the total cure rate reached 94%. 3 ineffective patients were treated by surgery.[Conclusion]The interventional therapy has the advantages of high success rate,less complication,less trauma,repeatability and low cost,it is a safe,simple and reliable means for gynecological therapy. This therapy is a new therapeutic approach for the patients with tubal pregnancy who accepted surgery or want to retain the their fertility.
