基于社区健康管理的慢性病综合防控策略构想

Thoughts about Strategies for Chronic Disease Integrated Control on the Basis of Community Health Management

在分析我国慢性病防控策略现状及问题的基础上,运用社区健康管理理论,提出一种全新的基于社区健康管理的慢性病综合防控策略,具体包括:慢性病防控主体及受体的主动性和积极性提高策略、社区居民电子健康档案的管理和利用策略以及社区慢性病防控的分阶段"干预包"策略,以达到提高我国社区慢性病防控效果和效益,提高社区居民健康水平和生命质量的目的。

Based on the analysis of the disease prevention status quo and problems, and using community health management theory, new strategies are put forward for chronic disease prevention on the basis of community health management, including strategy for improving subjects＇ and receptors＇ initiative and enthusiasm to chronic disease prevention, strategy for community residents＇ individual electronic health archives management and utilizatian, and strategy for phased intervention packet of chronic disease prevention interventions, to improve effect of chronic disease prevention strategy and the community residents＇ health and life quality.
