高龄老年高血压合并冠心病患者肾动脉狭窄发生率及危险因素分析

Analysis on the incidence and risk factors of renal artery stenosis of elderly patients with hypertension and coronary heart disease.

目的探讨高龄老年高血压病合并冠心病患者动脉粥样硬化性肾动脉狭窄的患病率和危险因素。方法对246例高龄老年冠心病伴高血压病患者进行冠脉造影诊断为冠心病的同时选择性双侧肾动脉造影。结果肾动脉狭窄的发生率为28.9%;高血压病合并冠心病组肾动脉狭窄发生率为33%;高血压病未合并冠心病组肾动脉狭窄发生率为7.3%。多因素回归分析中高脂血症、颈动脉斑块形成、严重冠脉病变和吸烟为肾动脉狭窄的独立危险因素。结论高龄老年冠心病患者应常规行选择性肾动脉造影。

Objective To discuss the morbidity and risk factors of atherosclerotic renal artery stenosis of elderly patients with hypertension and coronary heart disease.Methods To carry out bilateral selective renal angiography on 246 elderly patients with coronary heart disease and hypertension when they are diagnosed coronary heart disease by coronary angiography.Results The incident of renal artery stenosis is 28.9 percent;the incident of renal artery stenosis of the group both with hypertension and coronary heart disease is 33 percent;the incident of renal artery stenosis of the group only with hypertension is 7.3 percent;hyperlipemia,the formation of the carotid artery plaque,serious coronary lesions and smoking are the independent risk factors in multi-factor regression analysis.Conclusion Elderly patients with coronary heart disease should regularly undergo selective renal angiography.
