摘要 目的研究Ki-67、Bcl-2和EMA在膀胱肿瘤细胞中的表达及其与肿瘤细胞生物学行为的关系。方法用免疫组化技术(ISAB法)检测Ki-67、Bcl-2和EMA在膀胱移行细胞肿瘤标本中的表达。结果Ki-67标记表达率与肿瘤细胞的增殖率有关(除Ⅲ级肿瘤),P<0.01。在Ⅲ级肿瘤中用Ki-67标记率代表肿瘤的增殖活性不可靠。各种肿瘤参数中只有年龄因素与Bcl-2的表达水平有统计学上的相关性,P<0.05。EMA的表达率随着膀胱肿瘤组织学分级和分期的增加而降低(P<0.01,P<005)。在复发和多发肿瘤中EMA的表达率亦较低(P<0.01)。结论三种抗原均可从某些方面反映出膀胱肿瘤的生物学行为。但只有EMA与膀胱肿瘤的生物学行为关系密切,可作为判定膀胱肿瘤的恶性程度和愈后的重要标记物。 Objective To study the relationship between the expression of Ki-67, Bcl-2 and EMA and biological behavior of bladder tumor. Method Using an immunohistochemical method (LSAB), the expression of Ki-67, BcL-2 and EMA in formalin-fixed paraffin-embedded sections of tumor tissues were detected in 49 patients with bladder cancer. Result Ki-67 labeling index was associated with the proliferation of tumor cells in grade 0, Ⅰ,Ⅱ but in grade Ⅲ (P < 0. 01 ). It is unreliable to represent the proliferation of tumor by the Ki-67-positive incidence in grade Ⅲ. Statistical correlation existed between age factor and the level of Bcl-2 expression except for the other tumor background parameters (P < 0. 05). The frequency of EMA-positive cells was decreased with the increase of the tumor grades and stage (P < 0. 01 and P < 0. 05, respectively). Conclusion The expression of the three antigens can show the biological behavior of bladder turmor in some aspects. EMA is closely related to the biological behavior of bladder tumor. It is an essential marker for judging the malignancy and prognosis of bladder tumor.
