彩色多普勒超声对乳腺癌的诊断及鉴别诊断价值

Values of Color Doppler Ultrasound in Diagnosis and Differential Diagnosis of Breast Cancer

目的探讨彩色多普勒超声在乳腺癌诊断与鉴别诊断中的应用价值。方法回顾性分析经手术病理证实的68例乳腺癌患者及67例乳腺良性肿瘤患者的超声声像图,对乳腺癌二维超声声像图特征及彩色多普勒血流显像(CDFI)特点进行总结。结果乳腺癌与乳腺良性肿瘤患者的肿块二维超声声像图表现存在差别。CDFI示乳腺癌肿块内血流显示率为95.6%(65/68),乳腺良性肿瘤患者肿块内血流显示率为47.8%(32/67)。乳腺癌患者肿块内血流分级较乳腺良性肿瘤患者高(P<0.01);其动脉收缩期血流峰值速度(PSV)及阻力指数(RI)显著高于乳腺良性肿瘤患者,差异有统计学意义(P<0.01);且其肿块多表现为伸入性动脉血管(76.5%)。结论彩色多普勒超声在乳腺良恶性肿瘤的诊断及鉴别诊断中发挥了重要的作用;当彩色多普勒超声在乳腺肿块内部或周围,或异常回声区内探及丰富血流信号,流速曲线显示为高速高阻时,应高度怀疑乳腺癌。

Objective This study was designed to evaluate the characteristics of color Doppler ultrasonography（CDU） in diagnosis and differential diagnosis of breast cancer（BC）.Methods Ultrasonograms of 68 BC patients confirmed pathologically and 67 patients with benign breast tumours were analyzed retrospectively and the characteristics of BC two-dimensional ultrasonograms（USG） and color Doppler flow imaging（CDFI） were summarized.Results BC patients were different from those with benign breast tumor in tumor 2-dimensional USG.CDFI showed that the display rate of BC blood flow within tumors was 95.6%（65/68）,that of benign tumor 47.8%（32/67）.The blood flow grading within tumors was higher in BC patients than in benign breast tumor patients（P0.01）,and PSV and the arterial resistance index（RI） significantly higher（P0.01）.Its tumors manifested mostly as irruptive arterial vessel（76.5%）.Conclusion CDU plays an important role in diagnosis and differential diagnosis of benign or malignant tumors.When CDU probes rich blood flow signals within abnormal echo area or within or around breast tumors,and when flow curve is displayed as high-speed high resistance,breast cancers should be highly suspected of.
