Analysis of the causes of the unsuccessful treatment of endophthalmitis

目的探讨化脓性眼内炎治疗失败原因。方法化脓性眼内炎63例，6例采用传统的常规治疗，23例单纯玻璃体腔内注药，34例采用玻璃体腔注药及玻璃体切除术。结果显效13例占20．6％，有效15例占23．8％，无效35例占55．6％。结论未能早期诊断并在48小时内玻璃体腔注射抗生素及未切除玻璃体是眼内炎治疗失败的重要原因。药物敏感试验尚无结论者可选用头孢唑啉钠2．25mg玻璃体腔注射。

Objective To analyse the causes of the unsuccessful treatment of endophthalmitis and the effective methods in the treatment of endophthalmitis. Methods Treatment of 63 cases of endophthalmitis. Six cases were treated by traditional method,23 cases had intravitreal injection and 34 cases had intravitreal injection combined with vitrectomy. Results The visual acuity in 13 cases were more than 0.05 (20.6%),light percent (LP) to 0.05 in 15 cases (23.8%) and 35 cases lost visual acuity or eye balls. Conalusion Delay of diagnosis and intravitreal injection of antibiotic in 48 hours are the important factors cause the unsuccessful treatment of the endophthalmitis. Intravitreal injection of 2.25mg Cefazolinu Natricum should be of choice before the sentitivity antibiotics are known.
