槽形环髓内针治疗股骨干粉碎骨折的实验研究

摘要 对槽形环配合髓内针固定股骨粉碎骨折的方法进行了生物力学研究。选用防腐股骨10根,以Win-quist及Hansen股骨粉碎骨折Ⅳ型致伤。分别用梅花针和钢丝固定为钢丝组,实验后改用梅花针和槽形环固定为槽形环组。结果显示:加载1000N时,骨干长度的短缩改变和骨折线增宽的变化,槽形环固定骨折牢固程度明显优于钢丝组。槽形环的应力—应变测试,也证明在槽形环上不存在明显的应力集中现象。所以。 We had designed a slot loop steel foil(SLSF) and used it to fix comminuted femoral shaft fracture in marrow and studied its biomechanical feature.We selected 10 femoral bones antisepticized with formalin,and created them Winquist and Hansen Ⅳ type comminuted femoral shaft ftacture.The SLSF group:the fractured bone was fixed with Kuntscher′s needle and SLSF after diaplasis.Steel wire group:after the testing,the comminuted femoral shaft fracture was fixed with Kuntscher′s needle and steel wire.Results show that the femoral length was shortened on an average 8.73 mm,the width increased an average 0.71 mm when loading 1 000 N in the steel wire group;but the femoral length was shortened on an average 4.46 mm,the width increased 0.49 mm in the SLSF group.The femur was shortened more evidently in the steel wire group than in the SLSF group under the loading condition.The fracture line in the SLSF group was less widened than that in the steel wire group.Femoral stress test of the SLSF group gave values of each point,ie,Ⅰ577,Ⅱ380,Ⅲ310,Ⅳ385.5,Ⅴ358.5,Ⅵ257,and Ⅶ478 respectively with maximum loading.Conclusion:There is no obvious stress concentration on the SLSF,so any breaking of the SLSF owing to abrasion can hardly occur during the fixation of comminuted femoral shaft.The author suggested an ideal treatment of the fracture.
