中山市男男性行为人群艾滋病/性传播疾病危险行为及感染率现况调查

Survey on AIDS/STD Risk Behaviors and Prevalence Among Men Who Have Sex with Men in Zhongshan, Guangdong

目的获得中山市男男性行为人群(MSM)行为学特征及HIV、HCV和梅毒感染率,探索其HIV感染的影响因素。方法 2009年通过自愿咨询检测门诊和当地同性恋组织招募MSM进行面对面的问卷调查和血清学检测。结果共招募207名MSM,HIV抗体阳性率5.3%(11/207),梅毒血清学阳性率2.4%(5/207),无HCV抗体阳性检出。最近6个月,78.7%(163/207)的调查对象曾经与男性发生过肛交;其中与2人及以上的男性发生肛交行为的占49.7%(81/163);发生肛交时,自我报告每次使用安全套的仅占34.4%(56/163)。多因素Logistic回归模型提示,最近6个月,发生肛交的男性性伴数越多,其HIV感染的风险性越高(AOR=1.22,95%CI:1.04～1.42);已婚或同居的MSM HIV感染的风险高于未婚、离异或丧偶的个体(AOR=4.32,95%CI:1.09～17.04)。结论中山市MSM HIV感染率高,高危行为普遍存在,亟需采取针对性的干预措施。

Objective To know the HIV risk behaviors, HIV, HCV and syphilis infection rates among men who have sex with men （MSM） in Zhongshan, Guangdong, and to explore the influencing factors of HIV infection among them. Methods MSM were recruited through voluntary counseling and testing clinics and local non - governmental homosexuality organization in 2009. With structured questionnaires, face to face interview was conducted and the blood samples were collected for HIV, syphilis and HCV tests. Results A total of 207 MSM were recruited. The prevalence rates of HIV, syphilis and HCV were 5.3% （11/207）, 2.4 % （5/207） and 0, respectively. In the past 6 months, 78.7% （163/207） of the investigated MSM had anal sex with males. Among them, 49.7 % （81/163） had 2 or more male sexual partners, and only 34.4 % （56/163） reported consistent condom use during anal intercourse. Multiple Logistic regression analysis indicated that those who had more male anal sexual partners during the past 6 months （AOR： 1.22, 95 % CI： 1.04- 1.42） were more likely to be HIV sero - posi- tive, and those who were married or cohabiting （AOR = 4.32, 95%O1： 1.09- 17.04） had higher HIV prevalence rate than those who were unmarried, divorced or widowed. Conclusions A high HIV infection rate and high risk behaviors are observed among MSM in Zhongshan. Targeted interventions are necessary to prevent HIV spread in MSM.
