自制针刀结合伤科手法治疗肩周炎的临床观察

Clinical Observation on Self-made Small Knife-needle Combined with Traumatic Manipulations for Scapulohumeral Periarthritis

【目的】观察自制针刀结合伤科手法等治疗肩周炎的临床疗效。【方法】将40例病例随机分为治疗组27例和对照组13例。治疗组采用臂丛麻醉下自制针刀结合伤科手法等治疗,对照组采用传统针灸、推拿等治疗。全部患者均获得随访,随访时间为8～12个月,平均9.3个月。比较2组在临床疗效、肩痛症状解除及关节功能完全恢复所需时间及并发症发生情况的差异。【结果】(1)总有效率治疗组为96.30%,对照组为84.62%,2组比较,差异无显著性意义(P>0.05),提示治疗组疗效与对照组相仿。(2)治疗组治愈19例(19/27,70.37%),肩痛症状立即解除,关节功能完全恢复所需时间(4.47±3.08)d;对照组治愈7例(7/13,53.85%),肩痛症状消失时间(17.35±6.22)d,关节功能完全恢复所需时间(26.13±3.58)d。2组比较,差异有显著性意义(P<0.05),提示治疗组在解除肩痛症状及恢复关节功能所需时间上优于对照组。(3)治疗组无并发症发生,对照组并发症发生率为23.08%(3/13)。2组比较,差异有显著性意义(P<0.05),提示治疗组的并发症发生率低于对照组。【结论】自制针刀结合伤科手法治疗肩周炎疗效显著,值得在临床上推广应用。

Objective To observe the therapeutic effect of self-made small knife-needle combined with traumatic manipulations for scapulohumeral periarthritis（SP）.Methods Forty SP patients were randomized into two groups： 27 in the treatment group were given self-made small knife-needle combined with traumatic manipulations under the anesthesia of brachial plexus,13 in the control group were given conventional acupuncture and massage.Follow-up was carried out in all of the patients for 8 ～ 12 months,averaged 9.3 months.After treatment,the therapeutic effect,shoulder pain relief,time for the recovery of joint function,and the incidence of complications were evaluated.Results（1） The total effective rate was 96.30% in the treatment group and 84.62% in the control group,the difference being insignificant（P〈0.05）.（2） In the treatment group,19（70.37%） patients were cured,whose shoulder pain was relieved immediately,with the time for the complete recovery of joint function being 4.47 ± 3.08 days.In the control group,7 patients（53.85%） were cured,whose shoulder pain was relieved in 17.35 ± 6.22 days,with the time for the complete recovery of joint function being 26.13 ± 3.58 days.The differences between the two groups were significant（P〈0.05）.（3） Complications were not found in the treatment group,but the incidence of complications in the control group was 23.08%（3 /13）,the difference being significant（P〈0.05）.Conclusion Self-made small knife-needle combined with traumatic manipulations is effective for the patients with scapulohumeral periarthritis,which is practical in clinical use.
