导管直接溶栓治疗急性下肢深静脉血栓形成的临床疗效研究

Catheter-directed Thrombolysis in Treatment of Acute Lower Limb Deep Venous Thrombosis:A Study of Clinical Effect

目的探讨导管直接溶栓(CDT)治疗急性下肢深静脉血栓形成(DVT)的临床疗效。方法 21例急性下肢DVT患者给予CDT,溶栓导管外接微量泵持续注射尿激酶溶栓治疗,应用静脉通畅评分、静脉通畅率以及健患侧肢体周径差指标评估疗效。结果 21例患者给予CDT,治疗前后静脉通畅评分及健患肢体周径差比较,差异均有统计学意义(P<0.01),静脉通畅率为(52.6±14.1)%。治疗过程中无严重并发症出现。完成随访11例(随访率为52.4%),随访时间6～12个月,其静脉通畅评分及健患肢体周径差比较,差异均有统计学意义(P<0.01),末次随访DVT患者治疗后静脉通畅率为(63.4±12.9)%,深静脉瓣膜保存率为81.8%。结论 CDT治疗急性下肢DVT患者效果良好,且治疗是安全的。

Objective To evaluate the clinical efficacy of catheter-directed thrombolysis （CDT） in the treatment of acute deep venous thrombosis （ADVT） of lower limbs.Methods A total of 21 ADVT patients were given CDT.The venous patency score and the rate of venous patency improvement were evaluated by venography.The circumferences of healthy or affected limbs were measured during the thrombolysis.Results After CDT,venography showed that the venous patency improved significantly （52.6±14.1）%.No severe complications occurred during thrombolysis.The circumferencial differences in limbs were significant （P〈0.01）.Eleven patients were followed up for 6 to 12 months,with the mean rate of venous patency up to （63.4±12.9）%.The valvular function was preserved in 9 patients （81.8%）.There was significant difference in venous patency score and circumferencial differences （P〈0.01）.Conclusion CDT for treatment of lower limb ADVT is effective and safe.
