摘要
目的:探讨血清BNP结合胸阻抗法监测先天性心脏病(先心病)婴幼儿术后心功能。方法:随机抽取先心病患儿45例为实验组,按照心功能(纽约心脏学会NYHA分级)分为N1、N2及N3组。同期小儿外科住院患儿20例为对照组,术后1、6、24、48h测量左心室Tei指数,无创血流动力检测系统测量LVET及CI,采用ELISA法检测血清BNP浓度。结果:术后1h血清BNP及左心室Tei指数较术前轻度增高,术后6、24、48h血清BNP及左心室Tei指数较术前逐渐减低;术后CI逐渐增高,LVET逐渐减低。结论:血清BNP结合胸阻抗法无创血流动力学检测系统能够迅速、准确、动态地评估先心病婴儿术后心功能,值得临床推广。
Objective:To analyze cardiac function of infants with congenital heart disease(CHD) by BNP in serum and thoracic impedance methods after cardiac operation.Methods:45 infants of CHD were graded to experiment group randomly.Experiment group was graded to N1,N2,N3 group by grade of NYHA.Control group included 20 infants with pe diatric surgery disease.Serum level of BNP was measured by ELISA after 1,6,24,48 h of cardial operation.Tei-index was examined by Doppler echocardiographic at the same time.LVET and CI were measure with thoracic impedance meth ods at the same time.Results:The serum BNP and Tei-index of left ventricular increased after 1 h of cardiac operation.The serum BNP and Tei-index of left ventricular decreased after 6,24,48 h of cardial operation.The LVET decreased and CI increased after operation.Serum BNP,LVET and Tei-index were positively correlated with heart function,and had a negative correlation with CI.Conclusion:Serum BNP and thoracic impedance methods can associate quickly and accurately cardiac function of infants with CHD after operation.
