摘要 目的:探讨生殖细胞瘤的CT与MRI诊断方法。方法:应用CT扫描采用常规轴位扫描,以OM线为基线,层厚5mm,层距5mm,部分病例进行颅底扫描、冠状位扫描。MRI采用每位患者均行平扫及增强检查,平扫应用快速自旋回波序列T2加权像,自旋回波序列T1加权像行横轴位、矢位及冠状位检查;增强对比剂应用Gd-DTPA,剂量0.1mmol/kg,经肘静脉注射。结果:本组38例患者均行头颅CT检查显示,病变位于鞍区的11例表现为圆形、椭圆形混杂,边缘清晰,CT增强扫描可见病灶呈均匀强化,其病灶范围较平扫时略增加,同时行MRI检查,24例病变位于基底节区、松果体区及鞍区的患者表现为不规则形病灶,T1、T2加权像呈均匀等信号。结论:CT和MRI联合应用可以有效诊断颅内生殖细胞瘤。 Objective:To discuss the diagnosis methods of CT and MRI in intracranial germinoma.Methods:Some patients used routine axial CT scanning at the baseline of OM line,with a collimation of 5 mm and interslice gap of 5 mm,other patients were scanned in skull base with coronal scannings.For MRI scanning,all the patients were examined with plain and enhanced scanning.Plain scanning patients used spin echo sequences for T2-weighted images and spin echo sequences for T1-weighted images in axial,sagittaland corornal planes;enhanced scanning patients were injected with Uro grafin(0.5 mmol/kg) into ulnar vein.Results:For 38 patients with cranial CT scan,11 leisions located in sella area reveal circular or oval high density imaging with clear border.For 38 patients with MRI scanning,24 lesions located in pineal and sella area show irregular ones with intermediate signal intensity on T1WI and T2WI and intermediate enhancement on enhanced scans.Conclusion:CT and MRI can be effectively combined diagnosis of intracranial germ cell tumors.
