成人肾病综合征并发急性肾功能衰竭临床及病理研究

目的：探讨成人肾病综合征（nephrotic syndrome，NS）并伴发急性肾功能衰竭（ARF）临床、病理特点及预后。方法：收集2007年2月-2010年2月因肾病综合征并发急性肾功能哀竭八住的60例患者的临床资料，回顾性的分析其临床特点、病理特点、及其预后。结果：所有患者均以原发性肾小球病变为主要原因，其中老年患者多以继发糖尿病肾病、恶性肾肿瘤为主，老年患者的肾活裣病理主要以膜性肾病、局灶节段性肾小球硬化为主，而非老年患者的肾组织活栓病理类型多样；老年患者的发病率较非老年患者高，且老年患者的病死率高、治愈率低。结论：成人患者中，老年患者的病理类型主要以膜性肾病、局灶节段性肾小球硬化为主，其发病率高、治愈率低、预后较差；非老年患者的肾活检病理类型呈多样性，其发病率低、治愈率较老年患者高，患者的预后较好。

Objective：Discusses the adult nephrosis synthesis to draft （nephrotic syndrome, NS） and the partner becomes anxious kidney function failure （ARF） clinical, the pathology characteristic and the prognosis. Methods： Because collects in February, 2007 -- February,2010 the nephrosis synthesis to draft 60 example patient＇s clinical material which the concurrent acute kidney function failure enters, review analysis its clinical characteristic, the pathology characteristic,and the prognosis. Results： All patients take the primary renal glomerulus pathological change as the primary cause, in which old age patient many continue have diabetes nephrosis, the malignant renal tumor primarily,old age patient＇s kidney biopsy pathology mainly by membrane nephrosis, bureau stove stage re- nal glomerulus hardening primarily, but the non--old age patient＇s kidney organization biopsy pathology type is diverse^The old age patient＇s disease incidence rate compares the non--old age patient to be high,also old age patient＇s case fatality rate high, cure rate low. Conclusion： In the adult patient, the old age patient＇s pathology type by the membrane nephrosis, the bureau stove stage renal gtomerulus hardening primarily,its disease incidence rate high, the cure rate low, the prognosis is mainly bad; The non--old age patient＇s kidney biopsy pathology type assumes the multiplicity, its disease incidence rate low, the cure rate compares the old age pa- tient to be high,patient＇s prognosis is good.
