三踝骨折手术治疗27例临床分析

The Surgery Treatment of Trimalleolar Fracture in 27 Patients

目的总结三踝骨折的手术治疗方法和疗效。方法对本院于2007年5月～2009年6月27例患者采用切开复位内固定手术，随访观察其功能效果，分析切开复位内固定手术的适应症及优缺点。结果本组所有病例都得到有效内固定，术后获随访6～22个月，平均16．2个月。术后踝关节功能恢复良好，采用Kofoed评分标准进行疗效评估，优良率达92．3％，疗效满意。结论正确的术前判断、合适的手术入路、准确的骨折复位和恰当的的内固定是恢复踝关节功能的重要条件。

Objective To observe the surgical techniques and the effect of trimalleolar fractures. Methods From may 2007 to June 2009, 27 patients with trimalleolar fractures were treated with open reduction and internal fixation （ ORIF ）, which were reviewed retrospectively according to the advantage and disadvantage and options of internal fixation. Results All patients with trimalleolar fracture were treated by internal fixation effectively in the durations of follow - up ranged from 6 months to 22 months （average of 16.2 months）. According to Kofoed rating system, the good were found in 92.3%. The results are satisfied. Conclusion The correct preoperative decision and reasonable operrative approach,the precise open reduction and internal fixation are all very important to the restoration of ankle function affter trimalleolar fracture.
