卡维地洛、安体舒通治疗高血压疗效及对左室肥厚、心功能的影响

Effects of Autisterone and Carvedilo in Hypertensive Left Ventricular Hypertrophy and Cardiac function

目的：观察卡维地洛联合安体舒通及小剂量氢氯噻嗪（双克）治疗高血压疗效及对左室肥厚、心功能影响。方法：76例高血压左室肥厚病人给予氢氯噻嗪25mg／d，同时随机分为两组；治疗组：卡维地洛12．5—25mg qd，安体舒通40-60mg qd。对照组：给与美托洛尔12．5--25mgbid疗程均为1年。治疗前后采用超声心动图检查，比较2组左心室结构及功能变化。结果：两组均有明显降压作用，P〈0．05。治疗组比对照组疗效更显著P〈0．05，且有显著逆转左室肥厚作用，治疗后LVEDD、IVST、LVPWT、LVM及LVMI均明显降低，EF值、E／A均有明显升高，P〈0．05。治疗12月后，治疗组以上指标疗效优于对照组，P〈0．01，治疗前后对血钾、肾功、血糖、血脂均无显著影响P〉0．05。结论：卡维地洛联合安体舒通及低剂量双克降压，逆转左室肥厚，改善心功能疗效显著，且不影响电解质及血糖，血脂代谢。

Objective..To investigate the effect of autisterone Carvedilo and low dose of hydrochlorothiazide combination in the treatment of hypertension and left ventricular hypertrophy and cardiac function. Method：Seventy six patients with hypertensive Left Ventricular Hypertrophy（LVH） were treated with Hydrochlorothiazide 25 mg qd. Patients were randomly divided into two groups： The treated group（n=38）was given Carvedilo： 12.5--25 mg qd. combined with autisterone 40--60 mg qd; The control group（n=38）was given metoprolol 12.5--25 mg bid. Then the entire treatment lasts one ye, ar. All patients in the two groups were under taken ultrasonic cardiogram and then followed by comparisons of the shown left ventricular structural and functional changes be/ore and after the treatment. Results..BP（Blood pressure） was decreased in patients in both groups. （ P〈0.05）. As for the effect, The treated group is much remarkable than the control group（ P〈0.05） The treated group has much more impact of improve the situation of LVH, the reversal of Left Ventricular Hypertrophy including all the related parameters was significant in patients of the treated group comparing with the control group, 12 months after treatment, The figure above the treated group was better than the control group（ P 〈0.01） ,The combined treatment had no adverse effects on blood lipid ,glucose ,serum K＋ and renal function. Conclusion：Autisterone combined with Carvedilo and low dose hydroehlorothiazide was high effective for reversal of hypertensive Left Ventricular Hypertrophy with significant improvement of cardiac function.
