渗出性滑膜炎对机体免疫的影响

THE EFFECTION OF EXUDATIVE SYNOVITIS ON ORGANIC IMMUNITY

目的：为了解某些膝关节病变引起滑膜炎渗出与机体免疫功能的变化关系，我们对４６例膝关节滑膜炎患者检测了血清及滑膜腔积液免疫指标含量变化；方法：采用法国赛诺菲集团中国分公司提供的ＱＭ３００激光速率比浊全自动特定蛋白分析仪。测定患者血清及滑膜腔积液的体液免疫（ＩｇＧ、ＩｇＡ、ＩｇＭ）；补体（Ｃ３、Ｃ４）；前白蛋白（ＰＡＢ）；转铁蛋白（ＴＲＦ）；血清循环免疫复合物（ＣＩＣ）及可溶性白细胞介素α受体（ｓＩＬ－２Ｒ）等指标的含量，并与３９例正常人血清作为对照比较；结果：所有膝关节滑膜炎性渗出的患者，其血清免疫指标的含量明显高于正常对照组。同时对滑膜腔积液进行了细菌培养及涂片观察。发现，细菌感染阳性患者免疫指标的含量明显高于细菌培养及涂片阴性的患者；结论：膝关节滑膜炎性渗出是由感染细菌及机体免疫指标含量的变化而引起的一种滑膜组织病变。根据检测指标含量的不同，可为临床的诊断和治疗提供依据。

Objective:To know the relation between certain alteration articulation of knee cause synovium exudative inflammation and organic immunial funtion changes.Methods:Laser rate nephelometer was used to measure serum and synovia levels of humoral immunity (IgG、IgA、IgM、C s、C 4、PAB、TRF),serum circulate immune complex (CIC) and soluble IL 2receptor (sIL 2) in patients ,and compared with those of 39 controls.Result:The content of serum immunial indexes in patients with synovium exudative inflammation in articulation of knee were significantly higher than thoseint control group,bacteria stain smear discovered that immunial indexes in patients with bacterial infection positive were significantly than negative.Conlusion:Synovium exudative inflammation was a synovium tissular alteratic which resulted from infection and organic immunial function change .Determination of the content of immunial indexes is based on diagnose and treatment.
