摘要
目的了解强直性脊柱炎患者总体背痛程度与BASFI指教的影响因素,以期为该病的早期预防和控制提供基线资料。方法收集2007-07--2009-03安徽医科大学第一附属医院风湿免疫科268例强直性脊柱炎患者,对其年龄、性别、职业、学历、出生情况、病程、吸烟史、饮酒史、首发症状、HLA—B27等指标进行统计学分析,并运用Logistic回归对以上影响因素与总体背痛程度和BASFI指数间的关联性进行研究。结果268例强直性脊柱炎患者以青壮年(18~45岁)为主,男女比例为4.95:1,职业构成中以S-A和农民为最多,学历以初中勾主,首发症状主要为腰背部疼痛,吸烟患者占29.5%,饮酒患者占21.2%,HLA-B27总阳性率为94.8%。其中性别、痫程、学历、饮酒吏是BASFI指数的影响因素。结论针对女性患者、不同学历层次的患者以及病程较长的患者开展有关强直性脊柱炎知识的健康教育,普及饮酒对本病的危害,对于该病的早期诊断和预防,都有非常重要的公共卫生学价值。
Objective To know the influencing factors for total backache degree and BASFI Index of Ankylosing Spondylitis (AS) patients, and offer early prevention and basic information for this disease. Methods We collected 268 AS patients from Department of rheumatism and immunity, the first affiliated hospital of Anhui Medical University, and did statistic analysis for age, sex, occupy, education degree, birth condition, disease course, smoking history, drinking history, first symptom, HLA-B27, and used Logistic regression to research the association between such factors and the total backache degree, also the association between such factors and the BASFI index. Results The main patients were young and middle-aged people ( 18 ~ 45years) , and the male to female ratio was 4.95 : 1, main of 268 AS patients were farmers and workers, and the middle school patients had the greatest proportion. The first symptom were mainly the backache, and the smoking patients had the proportion of 29.5% ,the drinking patients had the proportion of 21.2%. The total HLA-B27 positive rate was 94.8%. The sex, disease course, education degree, drinking history were influencing factors of BASFI index. Conclusion We should carry out health education for female AS patients ,the long disease course patients and different education background patients. In the same time, we should popularize the hazard of drinking, in order to show significant public health value for early diagnosis arid prevention of this disease.
