腹腔镜结直肠癌根治术的并发症及其防治

Classification and management of complications of laparoscopic colorectal surgery

目的探讨腹腔镜结直肠癌根治术的并发症及其防治措施。方法回顾性分析该院2001年4月～2008年11月收治的235例次腹腔镜结直肠癌根治术患者的临床资料。结果术中及术后30d内并发症发生率19.4%,吻合口并发症发生率最高7.6%,其次为血管脏器损伤6.4%和感染4.3%,与辅助切口小有关的并发症6例(包括吻合口扭转、吻合口闭合、吻合口裂开),死亡1例;中转开腹率3.8%。结论腹腔镜结直肠癌根治术并发症以吻合口并发症最多,注意预防辅助切口小有关的并发症,适当的措施可以降低并发症发生率。

【Objective】To explore classification and management of complications of laparoscopic radical resection for colorectal cancer.【Method】The clinical data of 233 cases undergone laparoscopic colorectal surgery 235 times from Apr.2001 to Nov.2008 was analyzed retrospectively.【Results】The overall complication rates（intraoperatively and up to 30 days postoperatively） was 19.4%（46/235）,complications related to anastomosis occurred in 7.6%（18/235）,iatrogenic injury in 6.4%（15/235）,and there were 6 cases with complications related to small assisted in-cision.【Conclusions】Anastomotic complications is the most frequent during laparoscopic colorectal surgery,attention need be given to preventing complications related to small assisted incision,proper measures may reduce the morbidity.
