显微椎间盘切除术与椎板间隙开窗术治疗腰椎间盘突出症的比较研究

Comparison of operative result of MED with conventional iscectomy in treating lumbar disc heniation

目的比较显微椎间盘切除术(MED)与后路椎板间隙开窗术治疗腰椎间盘突出症的疗效。方法 863例椎间盘突出症患者随机分成两组,观察组为MED椎间盘镜组448例,对照组为常规后路椎板间隙开窗术组415例,观察两组患者手术时间、出血量、术后住院时间和恢复正常生活或工作的时间情况。结果 MED椎间盘镜组优良率94.5%,后路椎板间隙开窗术组94.2%,两组比较,差异无显著性(P>0.05)。但MED组在出血量、术后住院时间、恢复正常生活或工作时间优于后路开窗组(P<0.05)。结论 MED治疗腰椎间盘突出症总体疗效与后路椎板间隙开窗术相同,MED具有创伤小、出血少、术后恢复快等优点。

【Objective】To observe the difference between MED and conventional iscectomy in treating lumber disc heniation.【Methods】Eight hundreds and sixty-three patients with lumber disc heniation were randomly divided into two grups,one is the observation group with MED and the other is the control group with conventional discectomy.The clinical results were evaluated according to Macnab method and the blood loss,the operaton time,the post-operative hospital stay time and the time of recovery to ordinary work or life were observed.【Results】The excellent and good rates were 94.5% in observation group and 94.2% in control group.There was significant diference in all indices between two groups.【Conclusions】MED is as an effective operation as conventional discectomy for lumbar disc herniation.MED shows less trauma and blood lose and faster recovery.
