宫腔镜下输卵管插管通液术在输卵管性不孕治疗中的应用

The effect of hydrotubation by tube insertion on tubal infertility with hysteroscope

目的探讨宫腔镜下输卵管插管通液术在治疗输卵管性不孕中的应用价值。方法 2005年1月～2007年12月,选择输卵管性不孕症患者160例,随机分为两组(每组80例)。一组患者在宫腔镜直视下行输卵管插管通液术(简称宫腔镜组),另一组患者在全身麻醉下行宫腹腔镜联合输卵管插管通液术(简称宫腹腔镜组)。结果宫腔镜组24例患者输卵管通液通畅(通畅率30%),宫腹腔镜组57例通畅(通畅率71.25%),两者比较差异有显著性(P<0.05)。随访1年,宫腔镜组24例患者9例妊娠(妊娠率37.5%),宫腹腔镜组57例患者23例妊娠(妊娠率40.35%),两者比较差异无显著性。结论宫腔镜下输卵管插管通液术是一种临床可推广使用的方法 ,其中宫腹腔镜联合输卵管插管通液治疗效果最佳。

[Objective]To evaluate therapeutic value of hydrotubation by tube insertion on tubal infertility with hysteroscope. [Methods]160 cases of oviduct infertility women were chosen from January of 2005 to December of 2007. They were randomly divided into two groups （each group of 80 cases）. One group was only hydrotubation by tube insertion with hysteroscope （hysteroscope group for short）, the other group was hydrotubation by tube insertion with hysteroscope associateed with hysteroscope under general anesthesia （hysteroscope with hysteroscope group for short）. [Results]24 cases were succeed in hydrotubation by tube insertion in hysteroscope group （the successful rate was 30%）, and 57 cases were succeed in hysteroscope with hysteroscope group. The difference of successful rate between the two groups was significant （P〈0.05）. After one year follow-up, 9 cases succeed in pregnancy in hysteroscope group （9/24, the pregnancy rate 37.5%）, and 23 cases had pregnancy in hysteroscope with hysteroscope group （23/57, the pregnancy rate 40.35%）. The difference of pregnancy rate between the two groups was no statistical significance. [Conclusion]The method of hydrotubation by tube insertion on tubal infertility with hysteroscope could be widely used in clinic. The first-rank of hydrotubation method is by tube insertion with hysteroscope association hysteroscope.
