国产股骨近端锁定钢板治疗老年股骨转子间骨折疗效观察

Effect of domestic proximal femoral locking plate in the treatment of intertrochanteric fractures of femur

目的探讨应用国产股骨近端锁定钢板治疗老年性股骨转子间骨折的临床效果。方法对49例老年性股骨转子间骨折患者采用牵引床牵引复位后切开行锁定钢板内固定治疗,并评定疗效。结果 49例患者均获得随访,随访时间5～24个月,平均(12.6±3.7)个月;手术时间45～120 min,平均(60.3±12.8)min。术中出血130～550 mL,平均(235.8±42.3)mL;所有患者骨折均一期愈合,骨折愈合时间8～15周,平均(11.5±1.6)周;无髋内翻畸形、钢板松动断裂、螺钉切割或退出、感染等并发症发生。术后疗效优36例,良10例,可3例,优良率为93.9%。结论应用股骨近端锁定钢板治疗老年人股骨转子间骨折,尤其是合并大转子粉碎骨折者或骨质疏松者,具有手术创伤小、固定牢靠、骨折愈合率高等优点。

Objective To observe the effect of domestic-made proximal femoral locking plate in the treatment of intertrochanteric fracture surgery in senile patients.Methods Forty-nine senile patients with intertrochanteric fractures were treated with locking plate internal fixation after traction reduction,and the efficacy was evaluated.Results Forty-nine patients were followed up for 5-24 months,the average was（12.6±3.7）months.The operation time was 45-120 minutes,the average was（60.3±12.8）minutes.Blood loss was 130～550 mL,the average was（235.8±42.3）mL;Fractures of all the patients were primary healing,the fracture healing time was 8-15 weeks,mean（11.5±1.6）weeks.No hip varus,plate loosening or breakage,screw cutting or withdrawal,infection and other complications occurred.Among 49 cases,excellent in 36 cases,good in 10 cases,fair in 3 cases,the fine rate was 93.9%.Conclusion Proximal femoral locking plate for treatment of intertrochanteric fractures of the elderly patients,especially the patients with greater trochanter comminuted fracture or osteoporosis,it has the advantage of minor surgical trauma,fixed solid and high rate of fracture healing.
