2型糖尿病伴非酒精性脂肪肝相关因素分析

Analysis of the correlation factor of type 2 diabetes mellitus combined with nonalcoholic fatty liver

目的探讨2型糖尿病(T2DM)合并非酒精性脂肪肝(NAFL)的临床特征及相关因素。方法 224例T2DM患者根据是否合并NAFL分为T2DM合并NAFL组(136例)和T2DM未合并NAFL组(88例),检测2组患者空腹血糖、胰岛素和血脂,计算体质量指数和胰岛素抵抗指数并进行比较。结果 T2DM合并NAFL组患者的甘油三酯、胆固醇、空腹血糖、餐后2 h血糖、空腹血游离脂肪酸、糖化血红蛋白、天冬氨酸氨基转移酶、丙氨酸氨基转移酶、低密度脂蛋白-C、空腹胰岛素、胰岛素抵抗指数均较T2DM未合并NAFL组显著升高(P<0.01或P<0.05),而高密度脂蛋白-C较T2DM未合并NAFL组明显降低(P<0.05)。结论 T2DM合并NAFL与肥胖、脂代谢紊乱、胰岛素抵抗密切相关。

Objective To investigate the clinical feature and correlation factor of type 2 diabetes mellitus（T2DM）combined with nonalcoholic fatty liver disease（NAFL）.Methods Two hundred and twenty-four patients with T2DM were randomly divided into T2DM group（n=88）and T2DM＋NAFL group（n=136）.Some indexes including blood glucose,insulin,blood lipid,body mass index（BMI）and insulin resistance index were measured and compared.Results In T2DM＋NAFL group,the levels of triglyceride,cholesterol,fasting plasma glucose,2 hours postprandial plasma glucose,free fatty acids,glycosylated hemoglobin,aspartate aminotransferase,aminoleucine transferase,low density lipoprotein-C,insulin and insulin resistance index were significantly higher than those in T2DM group（P0.01 or P0.05）,but the level of high density lipoprotein-C was significantly lower than that in T2DM group（P0.05）.Conclusion T2DM combined with NAFL may have a close association with obesity,lipid metabolic disorder and insulin resistance.
