二尖瓣狭窄瓣口面积的超声定量诊断

摘要 目的探讨4种方法测量二尖瓣狭窄瓣口面积(MVA)的临床应用价值及准确性。方法对200例二尖瓣狭窄患者,均采用二维超声(2DE)法、二尖瓣血流连续性方程(MFCE)法、多普勒压差减半时间(PHT)法、主动脉血流连续性方程(AFCE)法等测量MVA值,并将其中180例手术患者的MVA值与手术中直接测量的MVA值进行对比。结果 2DE法、MFCE法、PHT法、AFCE法与手术实测的术中MVA值分别为(1.03±0.42)、(1.02±0.70)、(0.91±0.37)、(0.96±0.51)、(0.94±0.40)cm2,2DE法、PHT法、MFCE法与手术实测的术中MVA值相关度高,其中以2DE法相关性最佳(r=0.95),PHT法(r=0.86)与MFCE法(r=0.85)次之,AFCE法相关性处于末位(r=0.72),回归方程分别为y=0.95x-0.13、y=0.64x+0.25、y=1.32x-0.24和y=0.78x+0.21。结论 2DE法和PHT法操作简单、测量的准确性高,具有较高的临床应用价值。
